Threshold Therapy and Coaching, PLLC

Welcome to Threshold Therapy and Coaching, PLLC (“we,” “our,” or “us”). These Terms and Conditions (“Terms”) govern your use of our website, services, and related platforms. By accessing or using our website and services, you agree to these Terms and our accompanying Privacy Policy and Accessibility Statement. If you do not agree, please do not use this site or our services.

1. Nature of Services

Threshold Therapy and Coaching, PLLC provides two types of professional services:

  • Therapy Services — Delivered by a licensed clinical social worker (LCSW) in the state of Utah, subject to Utah Division of Occupational and Professional Licensing (DOPL) regulations, the Utah Mental Health Professional Practice Act, and HIPAA privacy and security standards.

  • Coaching Services — Focused on personal growth, life transitions, and goal setting. Coaching is not therapy and is not intended to diagnose or treat mental health conditions.

2. Eligibility

You must be at least 18 years old to use our services or website unless receiving therapy as a minor with appropriate parental or guardian consent.

 

Therapy clients must reside in Utah or another state in which the provider is legally permitted to practice. Coaching clients may reside outside Utah, as coaching is not a licensed mental health service.

3. No Emergency or Crisis Services

Our practice does not provide emergency or crisis intervention services.

If you are in crisis or experiencing thoughts of self-harm, please contact one of the following immediately:​

  • Call the Utah Crisis Line: 1-800-273-8255

4. Confidentiality and Privacy

All therapy services are provided in compliance with HIPAA and state confidentiality laws. Information disclosed in therapy is confidential except where disclosure is required by law (e.g., risk of harm, abuse reporting, or court order).

Coaching services are not covered by HIPAA, but all client information is kept private and used only for professional purposes, consistent with our Privacy Policy.

5. Scheduling, Payment, and Cancellations

• Appointments must be scheduled in advance through our secure scheduling system.

• Payment is due at the time of service unless otherwise arranged.

​

• Late cancellations (less than 24 hours’ notice) or missed sessions may be subject to a cancellation fee.

• We accept payments through HIPAA-compliant, secure payment processors (e.g., SimplePractice, Stripe).

6. Telehealth

Therapy and coaching sessions may be offered via telehealth platforms that are secure and HIPAA-compliant.

By engaging in telehealth, you acknowledge the potential risks related to technology (e.g., interruptions, unauthorized access) and agree to use a private, safe environment for your sessions.

7. Disclaimer of Professional Relationship via Website

Therapy and coaching sessions may be offered via telehealth platforms that are secure and HIPAA-compliant.

By engaging in telehealth, you acknowledge the potential risks related to technology (e.g., interruptions, unauthorized access) and agree to use a private, safe environment for your sessions.

8. Limitation of Liability

To the fullest extent permitted by Utah law, Threshold Therapy and Coaching, PLLC and its representatives are not liable for any damages arising from your use of this website or services, including indirect, incidental, or consequential damages.

9. Intellectual Property

All content on this website — including text, logos, graphics, images, blog posts, and materials — is the intellectual property of Threshold Therapy and Coaching, PLLC, unless otherwise noted.

You may not reproduce, distribute, or modify any content without prior written consent.

10. Links to Third-Party Websites

Our website may contain links to external sites for your convenience. We do not control or endorse the content, policies, or practices of these third-party websites and are not responsible for their accessibility, privacy, or accuracy.
